- What partnerships strengthen our work, and where do misalignment or barriers limit effectiveness?
TAKE ACTION

- What practices could be adapted in your system, and what conditions would need to be in place to support implementation?
- What leadership actions would be required to move this work forward in your context?
EVALUATE
- How do we know if our supports are coordinated and effective, and whose perspectives are included in that assessment?
- What evidence do we use to inform decisions, and how consistently is it used across the system?
- Where do we need to strengthen coherence, and what system-level changes would support this?
Moving Forward
